Primary Care Physician -SNF

About the Role

We are seeking a dedicated and compassionate Primary Care Physician to join our team, specializing in the care of patients in Skilled Nursing Facilities (SNFs). In this role, you will provide comprehensive medical care to elderly and complex chronic care patients residing in nursing homes and post-acute rehabilitation centers across TX, FL, NY, IL, and NJ.

You will work collaboratively with nurse practitioners, facility nursing staff, therapists, and social workers to develop and manage patient care plans, improve health outcomes, and ensure compliance with state and federal regulations. This position offers the opportunity to build meaningful longitudinal relationships with patients in a setting where continuity of care is paramount.

Key Responsibilities

Clinical Care & Patient Management

  • Provide primary and ongoing medical care for residents in assigned skilled nursing facilities, including assessment, diagnosis, treatment, and management of acute and chronic illnesses.

  • Conduct required in-person compliance visits per CMS regulations and state-specific requirements.

  • Perform medical assessments for new admissions and readmissions, writing orders for medications, treatments, and diagnostic studies.

  • Manage a patient panel, carrying a caseload of residents across multiple facilities.

  • For patients with complex needs, coordinate care with specialists, hospitals, and hospice services as appropriate.

Care Coordination & Interdisciplinary Collaboration

  • Participate in interdisciplinary team meetings, care plan conferences, and family meetings to coordinate care and discuss goals of treatment.

  • Collaborate with facility nursing staff, social workers, therapists, and pharmacists to ensure comprehensive, patient-centered care.

  • Work with case managers and transitional care teams to facilitate timely discharges and appropriate post-discharge follow-up.

  • Communicate effectively with patients' surrogate decision-makers and involved family members regarding care preferences and changes in condition.

Regulatory Compliance & Documentation

  • Maintain timely, accurate, and detailed documentation in the Electronic Health Record (EHR), including progress notes, orders, and discharge summaries.

  • Ensure compliance with all federal, state, and local regulatory requirements, including CMS guidelines for physician visits in SNFs.

  • Participate in quality improvement initiatives, performance improvement processes, and facility surveys.

  • Discuss and document advance care planning, advance directives, and goals of care with residents and their representatives.

Leadership & Coverage

  • Provide medical leadership within the facility, supporting the medical director in quality assurance and policy development as needed.

  • Participate in weekend, holiday, and on-call coverage on a rotational basis as required.

  • Supervise and collaborate with advanced practice providers (NPs/PAs) as part of the care team.

  • Obtain and maintain privileges at all assigned skilled nursing facilities.

Qualifications & Requirements

Education & Licensure

  • Medical Degree (MD or DO) from an accredited institution.

  • Current, unrestricted license to practice medicine in the state(s) of practice (TX, FL, NY, IL, or NJ).

  • Valid Drug Enforcement Administration (DEA) registration.

Certification & Experience

  • Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics. (Board Certification required within a specified timeframe if eligible).

  • Experience in a clinical setting, with experience in geriatrics, long-term care, or SNF settings strongly preferred.

  • Knowledge of SNF regulations, standards of care, and the CMS survey process.

Skills & Competencies

  • Strong clinical reasoning and ability to make critical medical decisions for a complex, elderly population.

  • Excellent communication and interpersonal skills for interacting with patients, families, and interdisciplinary healthcare teams.

  • Ability to work independently and manage time effectively while travelling between multiple facilities.

  • Proficiency with EHR systems and e-prescribing.
